Using SharePoint Online, I’ve followed the setup guide and got through with no errors.
There is no entry in the “Site Collection” features to enable the app.
I’ve run the SharePoint Shell commands with appropriate rights (sharepoint admin from root level) and installed the Forms App as per the instructions, but it’s not showing in the Site Collection features?
Am I missing something obvious?

Are you talking about Forms Designer (Classic UI) or Plumsail Forms (Modern UI)?
In either case, you don’t need Site Collection features for SharePoint Online.
For Plumsail Forms, please, follow this instruction.
For Forms Designer, please, follow this one.

There is no need to enable the app in the Site Collection features, it’s enough to upload package to the App Catalog and distribute it to all the sites. Once this is done, any form in your domain should be customizable, no extra steps required.
Tell us if you experience any trouble with the creation of App Catalog or with uploading the file. If you performed all the steps in the instruction, but still can’t customize SharePoint forms because of some error, please, send us a screenshot of the error.
