Custom fields in tickets

Hi guys

we currently have managed meta data that holds all of our clients and there hardware assets.

How can I add these fields into the tickets form… I have successfully added the columns to the list… but not to the form

please can you advise?

There is a button "Design Forms" in the Tickets list ribbon:
