Is it possible to modify the forms of a survey?

Any information on what the Surveys are in this case? Maybe you can link an instruction on how to create them from Microsoft? I'll test it, but the most likely answer is no, if this is the error shown.
We're on SP 2019 on prem.
It's not a big deal, but I was just trying to modify the build in survey forms.
Yes that’s exactly it. But for some reason the content type is not editable. Even with SharePoint Manager i can not overwrite the settings. I figured it just uses the same as a custom list, but something is different.
Edit. The reason I’m using this template is it comes with a setting to restrict access after submission. I guess i could alternatively create a custom list and remove contribute access with a custom workflow. The survey list just seemed easier.
