Sadly the support did not answer me since two weeks now. Form panels still show blank on every list solely in Chrome.
I couldn't find any new requests from you, I'm sorry about that.

I have a single user who is having this issue. They are not seeing my SharePoint form on load.
The user is missing the app package in the console as well.
Cache was cleared for Edge and Chrome browsers.
Not sure why it is a single user that is having the issue.

What exactly is the issue this user having? No forms in the panel, or no custom forms at all?
And what do you mean by missing the app package in the console? Can you post a screenshot?
The SharePoint form I created does not come up. It just goes to a blank page.
In your earlier screenshots you show the forms.plumsail.com in the console.
The user does not have that at all.

Are you sure you're using the latest version of the App Package? You can update it by following this instruction - Update the app package for Plumsail Forms (SharePoint Online) — SharePoint forms
Then, try to re-save the form and clear browser's cache (or try Incognito mode). Let me know how it goes!
All of that has already been done. I had did everything that was already in this thread.
I deleted the app package, installed it again new. Went into my form and resaved it.
Cleared the users browser cache for Edge and Chrome. Also did Incognito mode on both browsers.
I can see the app package in my console when I open the webpage with no issues.
The users page just opens to a blank white screen and there is no app package inside the console. It seems to be only 1 user that is having the issue.

Anything else different for the user? Network, ISP, antivirus software? You can open Network tab in the browser, this will record all requests by the browser, refresh the page with the Network tab opened and see if there are any requests to forms.plumsail.com that are blocked.
I now have a second user with the same issue. Below is a screenshot of their network tab and sources tab in the browser. They are being redirected to a blank page instead of opening a panel on the SharePoint form.

This is very important, please, click on the link in console to open the page, and then click on the Lock symbol in the browser near the URL to check for a certificate status:
This needs to be done by the user who has these issues.

This is a different user, no? You need to check it for the user who has the issue with the certificate, the one that gets this error:

I see now that you're checking certificate status for your SharePoint domain, that's not what's needed. You need to open the browser's console and click on the link in the error:

Both users are working from home just like I am.
