Are you using Microsoft Forms or Plumsail Forms? Are you using the modern or classic experience of the Microsoft Power Automate editor? Could you please share the screenshot.

Additionally, I recommend trying out Plumsail Forms. It is free, and is generally easier to use and offers more design flexibility comparing to Microsoft Forms. Plus, saving attachments to SharePoint is simpler and more direct with Plumsail Forms:
