Exception from 'Remove all permissions from item'

Suddenly our workflow actions 'Remove all permissions from item' throws an exception like this:

The workflows haven't been changed.

The issue only appears on workflows that is triggered ‘on create’. When the same workflow is triggered manually, it works as it should.

The problem arose because of a bug in the product. Currently fixed.
