Form sets keep disappearing

Is there a limit to the amount of form sets because when I add additional formset they seem to be created fine then in a few minutes disappear from the list?

Can you see form sets in the designer?

Can you open the form set in the browser? E.g. the link to the Edit form in the form set is the following:
{SiteURL}/SitePages/PlumsailForms/{List Name}/Item/{FormSetID}/EditForm.aspx?item={ItemID}

You can find the FormSetID at the bottom left in the designer
